
The Senior Assessment & Memory Program provides a comprehensive evaluation of an individual's overall well-being. By listening to your concerns and applying specialized skills in geriatrics, we are able to assist you in maintaining and improving your health.
Your comprehensive assessment will evaluate memory, medication, function, activity, ability, nutrition, and support. Family, caregivers and your physician are encouraged to participate.
When Memory Fails
When memory falters many things change and many questions arise for both seniors and their caregivers. Is this Alzheimers? Will memory return? Is living alone safe? Does anger come with memory change? Will this get worse? Where do I get help?
Don't Wait
An assessment for memory or function should be done at your first observance of any of the following:
- Increasing difficulties with memory
- A decline in function or abilities
- Depression or social isolation
- Increased number of medications
- Unsteadiness or falls
- Increasing need for care or supervisions
Who Performs the Assessment?
A Board Certified Geriatrician Physician, a Nurse Practitioner and a Registered Nurse make up your assessment team. These healthcare professionals are each specially trained to understand the unique needs of older individuals. Other specialists are called upon as needed, including rehabilitation therapists, neurologists, podiatrists and others.
Each member of your assessment team collects important pieces of information concerning your health and then meets to develop your personalized action plan. Your plan will provide recommendations for you, your care givers, and your physician.

Is There a Fee?
The assessment is covered by Medicare and most supplemental insurance carriers. We will be glad to file claims with them on your behalf. Please bring your insurance information at the time of your appointment.

Where is the Assessment Program Located?
The Senior Assessment Program is located on the campus of Bon Secours DePaul Medical Center at 150 Kingsley lane, Norfolk, Virginia. Parking spaces are available for your convenience. Wheelchairs are available to assist you. (Click on directions for animated map.)
Directions from I-64 East:
Take I-64 east to Granby Street (Exit #276). Travel 1/4 mile on Granby and turn right onto Painter Street.
Directions from I-64 West:
Take I-64 West to Tidewater Drive (Exit #277A). As you exit, remain in the off-ramp lane, turn right onto Thole Street (1st traffic light). Travel approximately 2 miles to Granby Street and turn left. Travel 7/10 mile, turn right onto Painter Street.
