That’s what the new Vital Net Telemonitoring System does it gives you the piece of mind of knowing that your blood pressure, heart rate, and other vital signs are within acceptable ranges on a daily basis.
Advances in technology allow home telemonitoring to give you more control over your own health care, as well as providing your physician with important information regarding your health status on a more regular basis.
If you suffer from heart failure, diabetes, chronic obstructive pulmonary disease (COPD), hypertension, or major depressive disorders, monitoring services will be given free of charge as part of your home health care. Once you are discharged from home health care, you can continue to be monitored daily for a reasonable monthly fee.
Telemonitoring is easy too! Each morning, simply use your telemonitoring system to check your blood pressure, heart rate, glucose, oxygen level and weight. After answering a few quick questions, a detailed report is sent to both you and the telehealth nurse.
If there are changes in your health status, the telehealth nurse will call to see how you are feeling and, if needed, she will arrange for a nurse to visit you in your home, and call your doctor to determine if your care plan should be adjusted.
